Rehabilitation of Edentulous Maxilla and Mandible with Microstomia and Limited Mouth Opening using Commissurotomy, Vestibuloplasty, and Implant-retained Overdenture: A Long-term Follow-up of 12 years

Abstract
We present a challenging case of microstomia that was restored with commissurotomy and overdenture placement. A 57-year-old female patient had febrile disease at the age of 6 years and had since suffered from limited mouth opening and teeth loss. An anterior crossbite with only two teeth in the maxilla and completely decayed teeth in the mandible were observed. The maximum mouth opening was 12 mm at the initial visit. Due to scarring and previous free flap in the left buccal cheek, the size of the mouth was very small. Under general anesthesia, full-mouth tooth extraction and commissurotomy were performed to increase mouth opening. Implant installation was performed in January 2011. Magnet attached overdentures were fabricated and delivered three months after implant installation. The patient was followed up for 11 years without any complications. Providing implant-supported overdentures for full edentulism has been suggested as an effective treatment option. Optimal surgical and prosthetic planning in patients with microstomia results in long-term success.
